The FMS application consists of seven pages and includes sections for personal information, education and work background, fellowship selection, and short answer questions. Your resume or curriculum vitae (CV) is required in order to be considered for enrollment. You will be asked to upload a digital version of your resume or CV as part of this application. 

Important: Your successful completion of the FMS training is required to be considered as a candidate for the FMS field assignment fellowship.
